It is essentional that you have experience/knowledge of:
- Managing intermediate care
- Patient transition from hospital to community
- Working in hospital and community in the NHS
- Engaging and establishing positive working relationships
- Service transformation
You must also have current HCPC registration & a right to work in the UK.
