Description

We are currently recruiting for an experienced Adult Community Physiotherapist to join a well-established NHS team in London.
This is an excellent opportunity to work within a multidisciplinary community service, supporting adults to maximise their mobility, independence and physical function, enabling them to remain safely within their own homes. The successful candidate will manage a varied caseload, carrying out comprehensive physiotherapy assessments, delivering rehabilitation programmes, and working collaboratively with health, housing and social care professionals to achieve positive outcomes for service users.

Key Responsibilities:
• Manage a caseload of adults with a wide range of musculoskeletal, neurological, respiratory and age-related conditions within the community.
• Work within the Transfer of Care Physiotherapy Team, undertaking discharge assessments and rehabilitation assessments for service users during the first six weeks following hospital discharge.
• Deliver holistic rehabilitation programmes to support recovery following hospital admission, promoting independence and reducing the risk of hospital readmission.
• Undertake comprehensive physiotherapy assessments in service users' homes and other community settings.
• Assess mobility, balance, strength, gait, transfers and functional abilities to identify rehabilitation needs.
• Develop and implement person-centred rehabilitation and treatment plans that support safe discharge, independence and community participation.
• Deliver evidence-based rehabilitation programmes, including exercise prescription, functional reablement and falls prevention interventions.
• Complete moving and handling assessments and provide appropriate recommendations.
• Work alongside the integrated health and social care team, contributing to coordinated discharge planning and ongoing rehabilitation.
• Delegate appropriate interventions to Therapy Assistants and provide clinical guidance and supervision where required.
• Provide regular feedback on patient progress during weekly multidisciplinary team (MDT) meetings to support effective care planning.
• Work closely with occupational therapists, social workers, nurses, housing teams, carers and other partner agencies.
• Undertake risk assessments and implement appropriate risk management plans.
• Maintain accurate clinical records, assessment reports and case documentation in accordance with professional and organisational standards.
• Participate in safeguarding procedures where concerns are identified.
• Provide advice and education to service users, carers and families regarding rehabilitation, exercise programmes, mobility, falls prevention and self-management strategies.
• Contribute to multidisciplinary meetings and service development initiatives.
• Ensure compliance with HCPC standards, CSP guidance and relevant legislation.

Essential Requirements:
• Degree in Physiotherapy.
• Current HCPC registration.
• Previous experience working within Adult Community Physiotherapy services.
• Experience carrying out physiotherapy assessments within community settings.
• Experience delivering rehabilitation programmes and exercise prescription.
• Knowledge of moving and handling principles, falls prevention and mobility equipment.
• Excellent assessment, communication and report-writing skills.
• Ability to manage a busy caseload and prioritise effectively.

Desirable Experience:
• Experience working within a London local authority or NHS Community Therapy service.
• Experience supporting hospital discharge pathways.
• Experience supervising or delegating work to Therapy Assistants.
• Experience working within multidisciplinary community teams.
• Full UK driving licence and access to a vehicle may be advantageous for some roles.
